Log into the admin dashboard using the link on the bottom of any page next to the copyright statement.
User the user and password supplied – it is case sensitive.
Choose the sidebar menu option HIPAA Forms
On the default page scroll down a bit to see ALL entries though you can sort them using the menus provided to only view one form or using various filters. You can view the entry inline, archive the data, download an encrypted copy, assign the submission to a particular user for follow up or delete the submission from here.
