The forms I made are using a plugin called gravity forms and a service called HIPAA Forms out of Wisconsin. Links to their company sites are on this page.
Quite simply to make the forms collecting patient data HIPAA compliant the data must be collected and stored on certain kind of server which is what HIPAA Forms does on their servers – to view the date you log into this site and it makes a connection to their servers to allow you to view the patient data.
This is meant to be a temporary, anonymous, storing of the patients data on the server. It is not intended or recommended for long term storage or management of that data which is why there is no user ID or Password required for patients to fill out the forms.
Right now there is only one form active using the free service – I set up the wart treatment form for review purposes but I have made almost all the forms in gravity forms so whichever you want to use we can turn on in a jiffy. All these additional forms can be added once PYAM has signed up for the service as the monthly fee covers unlimited forms and submissions.
